EZ-Pages TOC Sidebox - Version: 1.1

This mod (described here http://www.zen-cart.com/forum/showthread.php?t=157926) allows the topmost
chapter in a TOC to become the (clickable) header for the EZ Page TOC sidebox. The header page is also removed from the TOC list in the box below it to avoid repetition.
