Addon, form for Call me Now, Call back etc. - exist any?

I search any box or function, for this case.

Customer write some like this in any small form (side box):

Name*:
Phone Number*:
Preferred Date:
Preferred Time: anytime|am|pm|night
email:
URL from:
Question:

and eShop salesman get in Admin section any request and call back to customer phone...

If not exist, who have experience to make this? Write me PM. I use ZC1.38
