TOC only showing one item on page

Hello,

I am having an issue with the table on contents only showing the top most TOC item set in the EZ-Pages admin grid. I've included screen shots for clarity. I'm using a template from a third party, but disabled it to the default ZC theme. The issue was still present.

Anything I'm missing here? I've search high and low and can't find anything.

Those settings are pretty much the long and short of it?

It's a weird bug that only shows up with newer versions of PHP/MySQL/whatever. Fix posted here:
