Yes it was, I did a bit of fiddling and was able to reproduce the problem when the header.php was missing. So the problem would appear to have been in there.

Meanwhile, thanks to all who helped....

I replaced the entire includes/modules/pages/login/ folder which contains five files. This seems to have solved the problem.

In the English version:

the list of countries is in the Admin > Locations/Taxes > Countries.

The fields listed on the login form can be found in includes/languages/english.php
