Can someone provide a list of all contact Us form related files. My 1.3.8a installation is displaying my store information but no contact form below it. I think I may have overwritten a file or something while working on the site.
I looked through the configure section and could not find a settings to turn on the contact form.
Any ideas?

I think it would just be:
includes/languages/english/contact_us.php
includes/languages/english/html_includes/define_contact_us.php
includes/templates/template_default/templates/tpl_contact_us_default.php
