1.3.8a how do I add phone number to contact form

Saw message from 2005/06 on how to add phone number but nothing current. Are there directions out there for 1.3.8a on adding a phone number to the contact form.

Available from downloads section here - I haven't used it, but am guessing it is still current - you may want to check the support forum for this mod first.
