No form fields for new customer to register

Hi,

I'm not quite sure how to solve this problem but it seems that currently only existing customers who have an account can checkout from my store.

When going to check out the page looks like this:
http://kidzblankets.com/index.php?main_page=login

There is just a bunch of blank spaces where there should actually be forms that collect the email information.

Anyone know of any way to solve this problem?
