Conctact us Form with photo attach option

Hi...
I'm trying to install an option in my site that will allow the customer to send us an inquiry with a photo attached in the e-mail. Similar like the contact us form with an option to attach(or upload it to the server) an image to the message.
I'm a newbie in zen cart and I'm trying to build my site by customizing a template. Please let me know if any mod is available, any help or idea is strongly appreciated.

Find a html form with attachment capabilities and you can paste the html into an ez page

HI Kobra... Thank you for you reply.
I've already done some research and to be honest i've found few forms, but i'm not sure if anything is compatible with zencart and to be honest non of them didn't have any instruction on how to implement.
are there any zencart tested forms that i can try ...

Find or create the form code and paste it into an ez page
