Having a problem with Avignon Template

I installed 1.3.9f and tested using Classic Template and everything works.

Installed the Aivgnon template and i have a problem

On the Login/New Account page, in the Address Details section, the field title STATE/PROVINCE is missing. Also the dropdown for countries is not working.

I think a file got zaped in the transfer. Can anybody tell me what file formats this page?

Re-upload the template package with a good FTP client that reports issues
