Just as they would as they do now: Go through checkout, select their payment choice, and proceed.
They don't have to create a PayPal account to pay via PayPal.
Right now if they don't have PayPal then they're probably not clicking on the PayPal "checkout" button, and thus being taken through the normal checkout flow, selecting PayPal when it's presented, and making their payment there by simply using their Credit Card.

Ok, I'm in the process of setting this up and have a question. I don't ship to some countries, so what happens when I use the second option (the customer goes through normal checkout) then is taken to paypal and they select an address in a country I don't ship to? Is this selected address returned to zen-cart and overrides the previous address?
