Note: this has nothing to do with Zip Codes ... it is using the Zone Definition for:
Country: Canada
Zone: Manitoba

Make sure each of your Zones Definition on the DEFINE is properly defined ...

I have to admit, this is really making me very frustrated, I thought everything was working fine, but didn't know new problems keep popping up!

Thank you very much, your post enlightened me, I fixed the problem now: it was screwed up due to duplicated Zone Definition for AB and YT, once the duplications removed, everything works fine now.

But can you check why there would be a little amount added up to the shipping cost? it doesn't look like either GST or HST or PST, I have no clue where it comes from.

Look in the Configuration ... Shipping/Packaging ...

You need to set the Maximum Weight to: 20000 ...

You also need to set the Tare Rates to 0:0 ...

You can use ON: L3T 4G5 as the test postcode and the shipping fee is still showing $8.83 instead of $8.00
