Contact Us Form - add text box

I am a novice and would like to add a basic text box "How did you hear about us?" on the standard "Contact Us Form" above or below the message box. Basically, it would be a smaller version of the message box. Just enough space for people to fill in how they heard of us. I want to make it a requirement to submit the contact form and have the information appear on the email submitted.

Any idea of how to do this?

It makes more sense to have that on the Create Account page:

I haven't used it, and don't know if it works with Zencart version 1.5.0.

One issue I have with that mod is that input is mandatory. As a visitor, I find that annoying.
