States in U.S.A Problems and more

Hi all. Something is off in my new account sign up page. Few things:
1:
State/Province: Look at the states (I only use Unites states as country) there is Armed forces Europe Federated state of microniesia and im not sure all the states are there. BIG PROBLEM. Any ideas how to fix it?
2:
How can i remove the /Province from the State/Province?
3:
Where do i modify the Additional Contact Details (i dont want the phone there) i went to Customer Details but its not there.

You can remove the US territories you don't want to appear in the dropdown in Admin - Locations/Taxes/ - Zones

If i have missing states how can i add a state? just add a new zone?
