Just had the same thing happen to us on a 1.3.9h cart that uses PayPal IPN.
Received payment email from PP, checked account and payment is there, but it is not in the Admin of the cart.
In the PayPal log files, I am seeing a difference between the last order that did register in the cart and this one.
