Have you given each module a unique SORT ORDER NUMBER? Is ENABLED set to "True" ?

Be aware that COD only works on an order of Physical Products and not with anything Virtual such as Gift Certificates or Downloads ...

Hmmm, we really don't have a lot to go by now.
If I were in your position, the 1st thing I'd try is to enable *another* payment module. I suggest the check/money order (because there is so little that can go wrong with this module). Logic suggests that this probably won't show up either, but if it *does* we'll then be able to backtrack to see why this acts differently than the others.
If it *doesn't* show up, my next suggestion would be to temporarily revert back to the 'classic green' template to see if that has any effect. If that solves the problem then we'll be looking at a template related issue. If it doesn't solve the problem .... Hmmm.. lets wait and see before we go down that road.

ps. Don't forget to check your log files. These will often show the cause of many errors. Even those you aren't even aware of.
