how to create a custom form page for only registered users?

I need to create a page with forms to collect some information from registered users only. How do I do that?
Are there any addon or plugin can do that?

Where will you store this information??
You will need to create a table or records in an existing table
Create code to write this information to the table
Create the form
What do you expect to do with this information??? See it in the admin?
If so, then you will also need to create code to get the information and display it in the admin

I think I need to store it in the store database. I need to collect custom order information from customers.

Why not add a text attribute to the products that require custom order info

It is not about a product attribute, because I don't know which product the customer needs to buy, they must send in their info to let me know.

You could create a "product" that is an order inquiry/information-gathering device, with attributes for each bit of info they need to give you. Direct them to that as a way for them to make an inquiry.

See the festival site in my sig, on the Artists/Vendors > Vendor Application page, for an example.
