Add email form to page?

Hi! I need to add an email form to one of my pages. How would I go about doing this?

Doh...sorry.

I give my visitors an option to host an online "party" (you know...like a tupperware party...except online...and without the tupperware). I want to add a form where they can send their name, email address, date they would like their party to start, and any comments/questions so they can sign up to host a party. Being able to do that right from the information page would be so much easier and more convenient for them than having to email me.

BTW, I'm still running version 1.2.7

I can get the buttons to appear I'm just not sure how to go about getting a send button that includes username

Here's one way to do it ... I worked up this solution a long time back for this user who wanted to do similarly but with more information. Perhaps the concept will help you:
