I would like to create a customer survey form that is linked from the checkout success page and send that information via an email to the site admin. I originally was going to use the login/create account page form as the basis and take advantage of the authentication functions but have not yet had any success.

I have searched the forum extensively for custom form help and have only been able to come up with multiple references to a link from the old forum to instructions by DrByte. Unfortunately, the link does not work and my searches do not find anything.

DrByte refers to these instructions: "topic=6578" in this thread: http://www.zen-cart.com/forum/showth...highlight=form as well as a couple of other threads but my searches can find no link to this topic.

Does anyone know where to find this information or can you provide some sort of reference to the proper way to create custom forms that will email the captured data to a ZC store owner.
