Trying to Rent Products - Please Advise

Hi everyone,
I was hoping someone could help me with this.
I would like to set up a page very similar to this one:

Basically, I am planning on rent cell phones/handsets to people and I want them to be able to in put their phone delivery date and phone return date.
I also want them to have the option of selection insurance.

Here is another example ppage for what i want to do:

Can I do this with ZenCart? If so where do i start
Or should I just make a Page with a form?
