flat rate or table rate?

This is what Im trying to do:

up to $20 shipping = $6.95
$20.01- $30.00 shipping = $8.95
$30.01-$40.00 shipping= $10.95

would I use table rate and if so how do I set that up.

shipping from where to where? US to US or US to International or other way around, or both?

Zone rates might be better if all of the above and table rate good for in country only, because of the differences between domestic verses international rates.
