OK I figured it out. In fact it may have been the cause of the initial "error" on form load. When they added the new Express hold checkbox, they finally had another non-default value for the submit control. So this tells it explicitly to use continue button now.
Very simple fix:

find:
change to:Code:<input type="hidden" name="submitControl" value="">
Code:<input type="hidden" name="submitControl" value="Continue">
