Is there a multiple add to cart quantity box module?

I searched the downloads section with no luck. Then i searched the forums with not success either. What i want is to have 5 boxs at the bottom that are text input boxes were they can order 5 smalls 20 Mediums 3 Larges with out having to go back and forth from the shopping cart. I know the text input attribute works but how do you get that to add that number to the quantity order.

Did you get a solution to this? As we really would like this as a standard for our clothing products.

I found the below thread, it is regarding a module that is on the downloads, but for some reason has not been uploaded correctly. It only has the screen shots. In my opinion if the module was available for use it would be on my site already. The screen shots show that it is perfect for the multidimensional add to cart requirements that my site needs.

Anyway just thought, the link to the thread is for your information since I have already contributed to this thread.
