1062 Duplicate entry '25' for key 1 when trying to add to cart

I get this error when people try to add things to their cart.

1062 Duplicate entry '25' for key 1

It seems to add a blank entry in the cart ?

Site has been up and running for a year... now I get this.

Any help appreciated. - I have tried searching the site for info and found one thread that I did not understand - I am fairly new to this.

Have you more details on how you are making this happen?
