    Website Payments Pro *is* a credit card gateway, so will show you credit card fields the same way the Authorize.net module does. So, the "solution" to your "problem" is the same as described earlier: Click "Remove" on any modules you're not actually using.
    ie: if you've not subscribed to Pro, remove it.
    And, if you're using Express, then you should also click "remove" on Standard.

    It appears to be fixed. The customer said they had to turn off all but one setting in PayPal and now it works.

    I had the same situation and I turned off offline credit card processing and one set disappeared. I think one was for offline processing and one was for the website.
