Yes, it makes changes to the database .... ALL "SQL" scripts touch the database ... SQL is a database language.
Those settings shouldn't have any impact on PayPal.

I'm having the same problem with my site as well. I'm running v1.3.8, and when PayPal Express returns the user to my site, it's going to a blank page.
The interesting thing is that it appears to work when I refresh the page.
Any insight or help would be really great.

for me it was "Simple SEO URL" addon ...
to fix this i went into this modules settings, and as one of the values in the "Exclude List" i added ipn_main_handler.php
