Looking for a report

that includes these fields. We'd like it to be exported to a CSV file:
Date of Transaction
Item Purchased
Purchase Price
Qty
Sales Tax
Shipping Charged
Total of each receipt
Customer Name

If a customer purchases multiple items, it would need to list those.

Any ideas?
