Email Burst on change in order status

My client, for whom I administer Zen Cart, was updating an order from pending to delivered. The customer received 100 emails in a short space of time.

In looking at the database I see the records for at least 50 to the customer and numerous ones to the administration accounts (module order_status_extra). I have tried to replicate the problem on one of my orders with no success.

Has anyone seen this problem? Where else might I look for the cause?

What does the host say? Error logs?

There were no errors logged on the Apache server. The zencart database does have an archive of 50 emails to the customer that are identical. Those same emails went to the two email addresses I have set up to monitor zen cart activity.

No other indication that anything is wrong.
