No fields display on create account page

Hello - I have had a couple of customers create accounts in my store recently, but tonight I noticed that the create account page does not display any customer fields. I have searched the forums for a similar problem and haven't found one, so I'm hoping that someone has a suggestion. I have Checkout without account installed, but I have created accounts myself during and after the installation and testing of this mod.

I'm not sure where to start troubleshooting this problem.

I am going to start rebuilding on my test site from scratch, but if anyone has any suggestions in the meantime, I would love to hear them.

Thanks for the suggestion. The debugging utility did the trick. Although the error log didn't point me directly to the problem, it helped me identify the php file that was causing my error.
