Contact Us Link Returns Blank Page

I just received a report that my 'Contact Us' link is returning a blank page. I've been playing around with the site design and must have moved or deleted something.
The link is in the main page footer (includes/templates/site-name/common/tpl_footer.php) and was known to be working about a month ago.
It is still showing up under 'Define Page Editor' and may be edited there but returns nothing when the link (http://www.risquelingerie.net/index....age=contact_us) is clicked.
Sure would appreciate a clue for how to restore or get this working again.

In case anyone ever has this same problem. I am posting my workaround for this problem.
I created an EZpage with my contact info and replaced the "...?main_page=contact_us" link with my EZpage link. Now I have a Contact Us link that works but do not use the traditional Zen site email form.
Clicking our email link from the EZpage Contact Us now opens the users default email program.

Still would like to know what happened to the Zencart Contact Us form.
