Tested with Zen Cart® 1.5.5a, PHP Version 5.5.37
This version addresses some issues in the original version. The original was based on Zip Codes which can return some very erratic results for "local" deliveries.
This version calculates delivery “door to door” from the setting in the Configuration > My Store > Store Address and Phone to the valid delivery address.
