I have made use of MACHFORM by APPNITRO.
This is an amazing little piece of php/mysql software that will enable you to build very complex forms in just a couple of minutes. You need NO HTML or PHP knowledge as the form builder is a clever WYSIWYG interface.
The software gives you various options of embedding the form link into a page - and for EZPages I tend to use the <iframe> option.
THIS FORM (on an EZPage) took about 5 minutes to build and embed into the EZPage.
Data collected in these forms is both written to a little database, AND is emailed to whomever you want.

I looked at what Schoolboy posted and went for it.
Easy, awesome... I'm running out of adjectives to describe MachForm by Appnitro.
I have about no skills in php and still pulled it off. The interface could be used by a 2 year old. Then it's copy/paste in an EZpage, that's it!

I was having the same issue with getting a form to embed in an EZ page and I will say that the "match form" link given in this thread saved me.
If you get to this thread when searching the fourm for help, I agree with others here. spend the small amount it costs for match form. (unless you are totally zened) then you wouldn't be searching for the answer I would guess. It's so simple to use and the little db you build takes care of everything. I love this software!
