Japanese SAMA on Customer Documents

The cart is working fine in both languages except that the customer salutation SAMA (様) does not appear. It should be after the customers name in every document they see mainly the invoice, the packing list, the welcome email, the order confirmation email, and other emailings. The name order should also be LAST NAME FIRST NAME 様. Communication with the customer without SAMA appended is considered rude.

I'm just a simple shop owner and am having a devil of a time figuring this out.
