This isn't an ez-page. EZ-pages have the format index.php?main_page=page&id=
For index.php?main_page=contact_us you must enter a record in the database manually according to the section of the docs "Adding a URI Mapping for Other Zen Cart pages".

I did a fresh install of zencart 1.39h and installed the ceon url addon. I renamed my admin and installed all of the files per the instructions. The ceon_url_mapping.php file that was in the files folder....admin/includes/functions/extrafunctions/ceon_url_mapping..... Once I install this file my admin is no longer accessible. I get a 404 error. When this file is deleted, my admin is back. Any help is appreciated. Has anyone had this problem?

It sounds like you renamed your admin folder but forgot to update the value of DIR_FS_ADMIN in admin/includes/configure.php

We've tried to install your fine add-on on a webshop testpage that we have, but it doesn't work completely.
The thing is that the quality of the URLs changes. If we for example try to make an URL called
www.greatshop.com/lego-soldier, the URL quickly changes to a more SEO unfriendly link like this: http://www.greatshop.com/lego-soldie...idvh123fghr12#
Maybe we need to put .php in the end of all our links..?

Yes, they have.. if you had have searched the thread you would have found out more.
Zen Cart adds its ID for static URIs when first loading as it doesn't quite set its cookie path properly. We hope to fix Zen Cart to work better with static URIs using new code in version 4.0.0 of the module.
However, the zenid is ignored by search engines so you can just ignore it too.. once the first page that has a zenid has been clicked on the all your URIs will appear as you expect without the zenid.

Do you know when the next version will be available? A lot of us would definitely like to try it out in a BETA version of some kind :-)
Anyway, I don't get this:

It hasn't happened to our page yet, but maybe because we use the same 2 or 3 IP addresses always?

When I click on www.e-boger.dk the links to the categories are like http://www.e-boger.dk/digte-og-lyrik?zenid=asdasd Once I click on http://www.e-boger.dk/digte-og-lyrik?zenid=asdasd all the links no longer have the ?zenid=asdasd appended. This is as expected.
If that's not happening for you it's something weird with your browser.

Well, could be my bloody browser. .
Which one did you use to look it up?
I tend to use Firefox with Google accounts and browser history turned on.
I better go and erase the cookies in my system!
