Change the top one so it is active like this
See if that will work for you, and make sure you clear your cache in admin each time you make a change to simple seo.
Just tried it and had no luck.

I have this running perfect on zencart 1.3.9f. I found one file that was causing a conflict. Look in includes/functions/html_output.php
scroll all the way to the bottom and add a and see if that don't solve the problem. Make sure you clear the cache in admin.

Let me ask you this, is mod_rewrite enabled on your APACHE server (which is true in most cases) Are you running PHP 5
Is your site in a subfolder.

Hi, yup mod_rewrite is enabled, php 5.2.9, no the site is in the base folder. This is a local wamp setup. Prior to upgrading it was working great. I am uploading a fresh setup to my hosted setup to see if that works differently.

Hi thank you for the update and letting us know it's working on your online server. I didn't know you were testing on your home server. I use XAMPP for Windows to test before I upload live. Don't test Simple Seo on your home computer server. It won't work right and causes all kinds of problems. If you going to test simple seo prior to uploading set up a sub-Domins to test.
Re: Simple SEO URL [support thread]
Hi,
Mine is still not working and i have installed it on a live server. But the strange thing is that om my local host i have the same problem.
