I have done all that and still nothing wouldn't pull up in the phpMyAdmin and it still showing in the admin -> configuration

You are surely doing something very wrong. Since I don't have access to your phpMyAdmin I don't know how to help you. Sorry:blink: All should work what I have mention above.

well I found them and deleted it 5 times and it keeps installing it back into the Database. Why is it doing that?

Have you removed....
1. Revert the changes made to html_output.php
2. Remove all the Simple Seo files uploaded during the installation?

includes/functions/html_output
If you done all that I have no clue why it is reinstalling itself back.

I have re-upload the original html_output.php and in then go into the database and delete the sql and wait for a while and see if that took care of it.

I have did everything,,,, when I try to delete it from the database it came back there was an error:
MySQL returned an empty result set (i.e. zero rows). ( Query took 0.0075 sec )
then when I hit refresh I also go this error:
#1064 - You have an error in your SQL syntax; check the manual that corresponds to your MySQL server version for the right syntax to use near 'LIMIT 0, 30' at line 2

I have no clue whats going on since I can not actually look at it myself. Sorry again.

I'm using zencart 1.39h,I used Ultimate URL SEO before.
I follow the instruction to remove it,and install module manager,then the Simple URL SEO.Everything is right,except that I click on whatever generated url and it just returned to homepage only.
Does anyone have a clue on it?
