Anyway, the steps that worked for me and garyclemens:
Remove paypal ipn (uninstall)
Update the patch if needed.
Reinstall, set pdt token as instructed by garyclemens above
Also turn on ipn and auto-return in paypal

Yellow I setup the pdt token as instructed by garyclemens and had my friend make another purchase. It still does not show the info in admin or send me a email. At least paypal does. I just don't know whats happening here. Just had a thought. I have my main store set up for paypal express not Paypal IPN. Do you think this could be the problem? I don't know. What if I set the other store for paypal express instead of IPN you think that would resolve the problem.

Wait, lets clear things up first:
1. so the store you are testing right now, is it using paypal ipn or paypal express?
2. can you turn on the debug log in admin, then pm me the log after you placed an order?
3. also, pm me your store address. And if possible, the store admin so I can peek into the configuration (this is just optional, we can still work it out without that)

Yellow the store I'm trying this on is set up using ipn. My main store that this site is going through is using paypal express. I can not send that log to you right now, my friend will not be on for a few days so I have no one to purchase from me right now. I have the item setup for $1.00 if you would go there and make a purchase and I will refund your money then I will send you the debug info and I will pm you with all the rest of the info.

And the store url is? And you did reinstall the paypal ipn right?

Did you try the ipn check script that goes with the 1.3.8 zen default package?
