Not sure about the medium issue BUT You have the same issue regarding the large image, Image handler isn't installed correctly, when you click on the image and the larger one appears in the pop-up, the large image isn't water marked because it isn't going through image handler.
I don't know what browser you are using, but when you open the pop-up large image, right click on it and select image properties and it will teel you

is the image that is appearing
if imagehandler was working on the image, it would say

are you putting the popup-image file included with image handler up (I think thats the one, someone will need to correct me)

Image handler is working something is not correct in the pop-upcode
I believe there should be 6 file conflicts
1 in admin
and 5 in the includes
Store_Root\admin\includes\modules\category_product_listing.php
Store_Root\includes\modules\pages\popup_image\header_php.php
Store_Root\includes\modules\pages\popup_image_additional\header_php.php
Store_Root\includes\modules\additional_images.php
Store_Root\includes\modules\main_product_image.php
Store_Root\includes\templates\template_default\popup_image_additional\tpl_main_p age.php

just another thought:
1. go to your server files and set your ftp client to allow you to see invisible files
2. check the bmz folder and see if you have a file called .keep
I do not have one in my installation and the thing works as it should but others have stated they do have it so...if you don't have one create it in a text editor. just creat an empty doc and save it as .keep then upload it to bmz_cache folder and see if that works.

Nigel, you're absolutely right: there are six. (I really need a better file compare/merge tool; that's my excuse and I'm sticking to it.)
I replaced all six with the IH2 version, and I'm now getting watermarks in all the desired locations.
I'll try to replicate this in my shop, and will report back.
One lingering issue, though...by taking the IH2 versions of these files, am I throwing away any v139 modifications that will eventually be necessary? A nice addition to the IH2 installation notes would be a description of how to merge these six files.
Anyway, this is the biggest progress I've made in ages. Thanks to EVERYONE for the help.
Update: I recopied the six files mentioned by Nigel above from IH2 into my shop. Watermarks are now WORKING! I'll get a chance to test it some more after I add some new products, but I see no reason it shouldn't continue to work with images imported with Easy Populate. Lightboxes are also watermarked. All is good.
I still have a lingering concern about "orphaning" the changes introduced to those files in v139, but I guess I'll deal with that if and when something comes up.

Thats my issue at the moment, i am trying to work out what is imagehandler code and what is zen cart code so i can merge the changes into 1.3.9a versions of those 6 files.
On the whole there don't seem to be major differences in some of the files, whereas others do seem to be well and truly changed, also i want to make mine php 5.3 compatible, so i am kind waiting to see what diva Vocals comes out with, as i have kids underfoot, which doesn't help with code comparisons

Your reputation was never on the line IMHO.. I had legitimate questions, and if you assumed that your reputation was on the line for asking legimate questions, there is nothing I can do about that..
