Your images show up fine.
Your med image is being called normally and correctly. The webpage is loading your MED image.
I suspect you have dimensions set incorrectly in Admin->Images, causing it to be sized unexpectedly.

There is a 2nd image though that doesn't show up at all. Before I tried the sm, med, lrg files I would just name the image & then for the 2nd image it would be xxx_01.jpg. The second image would then show up on the product detail page at the very bottom. Now, I can't get that to show up.

That's because you don't have a "small" version of the 2nd image uploaded.
You need to put a 75608432_01.jpg file in your /images folder.
Then it'll know to enumerate all the "extra" images for that product, and then will look for larger versions of those "extra" filenames.

Thanks for the recommendation of this software - Irfanview (free). It made the renaming the all the image files I've already optimized very quick!

Hello all
Ok I am setting up my images and I want to make sure I start off on the right foot. Here is what I have:
1. /images/custom (this is where I put the small pics)
2. /images/medium/custom (for _MED.jpg images)
3. /images/large/custom (for _LRG.jpg)
Is this correct?
Secondly I have about 500 product pics that are .gif images, will they also work with this system?

you can use .gif as long as ALL your images are .gif 's
