Zone rates incorrect calculation
For a Canadian store I am calculating the following:
<$100 = $40
>$100 = $20.
Everything is coming up as $40 to the USA instead of the shipping price changing to $20 when the order value increases past $100. Here are my settings for this zone:

This formula says:
For prices UP TO 99, charge 40
For prices UP TO 100, charge 20
If EVERYTHING greater than 100 gets the $20, then set the upper limit on the price component to something ridiculous:-
99:40.00,100000:20.00
(You don't have to set it to a googol, but just a few zeros)

It's still charging $40 when it should be only $20.

Do you have another shipping module active? And if so, is there a $40 amount there somewhere?

I'm using one for Canada which is seperate and works great. No the rate there is $20 flat rate, or free if over $100.

I removed all modules several times.
My conf was this:
ZONE RATES ... used for USA only
FREE OPTIONS ... used for Canada Only
STORE PICKUP ... used for local provincial zone only
FLAT ... used for Canada Only
The solution was that I had to emoty the cart and LOG OUT to make it work right.

Hello, I am setting up a site for the online selling of wine for a friend of mine in Italy but the main clients will be american so I am setting all the price for ITEM but from 1 to 15 bottles is everything fine but for bigger purchase, from 16 up I get always the same amount (300 Euro), that it is wrong.
Here is my table:
