Zonetable shipping module

I've installed the zonetable shipping module for a Canadian store. Everything works great for orders in Canada, but now the international orders do not show any shipping options at all and the US orders show zone rate is active, but no price.

Apparently, I have to manually enter all the zone definitions for each country in the world, except US and Canada. However, I found a great sql patch that was made for all countries except the US. (found in thread: Shipping and Payment Modules >International Shipping )

Any chance anyone has made a patch for all countries except Canada? Or some other option that might work for me?
