Looks to me like there is a <p></p> in your live chat sidebox file that should not be there that is creating a bit of extra space at the top.

p, address in your css is giving you the space thanks...

So it seems the customer details are being entered fine, just not the order and totals.

If you are using Microsoft Outlook, the images cache are saved in the same place as Internet Explorer so you may need to delete the IE cache (if you are using Firefox perhaps).

I had the same problem.

IE caches any images that you view on the web. Make sure you delete the cache (I'm using Firefox but I like to test in a couple of browsers)

Also, Microsoft Outlook...

Configuration>Max Values>Manufacturers List - Scroll Box Size/Style to 0 and it worked for me.
I now have a neat little drop down.
