No record of the order in the admin panel, or on the order history. ...

The admin/includes/configure.php is not.

I'll make the appropriate changes and let you know if this fixes the problem.

Can the "always free shipping" not be used with these modules? I like having the graphic come up for "free shipping".

It was in use for 3 months at this site without any problems. Now I can't...

But now the "store pick-up" option isn't coming up anymore.
