The trouble with that is, the image is listed in the CSV, but there's no corresponding image file - coupled with the fact that most of the items that have images missing seem to be discontinued stock...

First thing I would check is that you haven't got any blank lines at the end of any files you have edited. Dreamweaver for example, is notorius for doing this (though I've never experienced it myself...
