I'm not a flash expert, but in the source code you will see the root to the folder containing the flash.

I have checked the following folders for files related to contact us and they look ok compared with a working site.

includes-templates-theme213-templates...

My problem was my own fault...
I added the
additional_images.php
outside of my template override folder.
moved it inside & bingo!:clap:

Might be worth double checking.
