If you get the error message without any rewrite rules, then clearly there is a major problem with your mod_rewrite installation or setup.

MZMT uses standard Zen Cart functionality to display the number of boxes and weight. You can change how this is calculated and displayed under Admin -> Configuration -> Shipping/Packaging
When you change the number of zones, you need to remove and then re-install the module from the Admin Shipping Modules page.
