Sorry I wasn't clear. USPS, UPS and Flat Rate options are used for Domestic. The flat rate only works in the United States Zone. So UPS and USPS are used for both domestic and international.

I don't think so, if I enter a handling charge there it will charge it to domestic (USA) customers too. It is only applicable for orders outside the USA.
