Customer Referal Box

How do I make a customer referal box at checkout so that if someone refered a new customer to my site I can give the refery a gift?

I don't want it to be a have to fill it in box, just optional.

What happens when the customer makes their second order. Are you going to give the refery another gift? Having a referal box at sign-up makes more sense.

Yup that is what I was looking for! My question is though, what is setting 1 for? I am looking to simply tell all of my customers that when they refer someone to enter the persons name in the box, then I will send both of the customers a coupon or gc. Does this make sense?
Using this setting how do I do that?

One more thing, I tried to test it, here's what I did:
set to 2
created coupon named referal
then I created an account entered referal in the referal box and nothing happened, I didn't get the $5 or anything, what am I missing?

okay thanks, I am thinking it doesn't really do what I want it to....
