Create a Catalog Request Form

I want to create a Catalog Request Form for the user to fill out.

How can I accomplish this.

Any help will be very much appreciated.

You could use an E-Z page or the blank sidebox mod and insert your HTML form code there.

This would be entirely separate from Zencart, so the customer would need to re-enter address info, etc.
