need some help regarding attributes and quantity per attribute

hi
i wish to customize my product info page to show diff fields for different attributes

for eg if a customer wishes to order 5 quantity of attribute 1
and 10 quantity of attribute 2

i wish to show them in a table with an add to cart box at the bottom

I tried it but nothing happened.
I uploaded the file in my server which was not in my server.
And merged those file which already exists in my server.
