Nope, just tested it and made a purchase in my own shop, it still happens!
I'm directed to page 2 of the checkout with the nasty red message, and my cart contents are still intact (like I never paid). But my order shows up in my order history, I get a confirmation email of my order and payment. Then if I log out, then log back in, my cart clears.
If I were to install a brand new uncustomized Zen Cart site using v1.3.8a, what would I have to do to it in order to configure it exactly like your store, so that I could see the exact same problem happening?

I installed a brand new version of 1.3.8a
I use Paypal IPN and Money order for shipping.
I've cloned the flat rate shipping module (so I have 3 flat rate shipping options). I also installed a new zone (all destinations except US & Canada).
I installed an editble sidebox mod.
I use the multiple images option (but have the images vertically instead of horizontally).
I offer regular products aswell as downloads, and of course Gift Certificates.
I removed the Gift Certificate redemption code box from the checkout_payment page, to force users to enter the code on the Gift Certificate FAQ page instead (I didn't like how you had to enter the code on the checkout page, then back out of your cart and go back in, in order to apply the amount -- it was too confusing, so I just removed the redemption code box from that part of the page and redirect them via a clickable link to enter it on the FAQ page then go back to their cart where they could apply the amount).
I've done a bit of customizing to the template (colors, table width, changed some working in the language files).
I really can't think of anything else I've done.
Any suggestions?
Having just done a fresh install of Zen Cart, set up PayPal IPN, sent myself a Gift Certificate, redeemed it, and completed a checkout properly without the symptoms you describe ... no, I have no suggestions.

Do you have any taxes set up? If so, do taxes apply to the orders for which you're encountering odd behaviors?
